i.   Company legal document

     a. Company Deed and its Amendment (if any), and the Letter of Approval from Minister of Justice & Human Rights of the Republic of Indonesia

     b. Tax Number (NPWP), Trade Business License (SIUP), Company Registration Certificate (TDP) , and Certificate of Domicile

ii.   Identity card of the Commissioners / Directors/ Legal Owner

iii.  Active bank account for the last 6 months

iv. Copy of on – going projects’ contract

v.  Sale and purchase agreement with the suppliers / sales note for the items to be leased.

vi.  Brochure or catalog (for machinery application)
